New Patient Info & Forms

All visits are by appointment only. We will only see patients younger than 14 years of age if they have previously been seen by a pediatric rheumatologist.

We begin our office hours at 7:30am to help accommodate our patients who begin their workday at an early hour. If you need to cancel an appointment, please notify us at least 24 hours in advance so that we may schedule others who are waiting for appointments. If you fail to make your appointment, a fee will be assessed. If you fail to keep your initial visit or cancel less than 24 hours from the time of your appointment, no future appointments will be provided.

Please complete the Medical Questionnaire and bring to your appointment along with the following items:

• X-ray films/discs

• Diagnostic studies

• Laboratory reports

• Insurance card(s)

• Picture ID

• Any office visit copays
